A new W9 will require the following fields:

First & Last Name
Address, City, State, Zip Code
Business / Personal indicator (business_personal)
TIN Type and TIN (EIN or SSN)
Signature

Business entities (business_personal = 'B') require the following:

Business Name
Tax Classification

Language

Credentials
